Complete your evaluation form and obtain CE Credits

Instructions on Accessing Evaluation and Certificates for CE Credit:

In order to access the evaluation and retrieve your CE certificate, please click "Evaluate Activity and Earn Credit" on the main CE Center site (www.snmmi.org/cecenter), and do one of the following:

• Option 1: Login using your normal SNMMI login and password (nonmembers that have purchased online courses, or have been members in the past may know their passwords; generally, SNMMI ID is your initial login and Last Name is your default password) If you are able to successfully login using Option 1, please proceed to the evaluation.
• Option 2: If you are unable to login using Option 1, use Option 2 – Registration ID and Last Name. Select "2013 SNMMI Mid-Winter Meeting" from the drop-down list and proceed to the evaluation.

If neither Option works, please enter your E-mail above and attempt to reset your password. If you are still unable to login, please send an email Memberinfo@snmmi.org, and Cc education@snmmi.org. Indicate your SNMMI ID, and request that your password be reset. Once you have received your login credentials, please login using Option 1 at www.snmmi.org/cecenter.
