Registration
You must register to receive credit for attending the sessions. If you are only attending SNMMI governance meetings, and not attending the educational symposium, no registration is required.

If you wish to attend, you may register onsite in New Orleans, beginning January 23, 2013.

**Residents, graduate students, interns, in-training, and technologist students must provide a letter on institution letterhead signed by your program director indicating your status to receive reduced registration fees. This letter must be submitted along with the registration form. Without this letter, you will have to pay the full registration fee.

Registration Cancellation/Change Policy
Written cancellations received by midnight, Thursday, December 13, 2012 EST and are refundable less a $50 processing fee. Cancellations
made by mail must be postmarked by December 13, 2012. After December 13, 2012 cancellations will not be refunded and are subject to the entire meeting fee. Please note that if
you do not cancel and you do not attend the meeting, refunds will not be processed. Substitutions may be made at anytime.
SNMMI will not be held liable for cancellation fees to
hotels, airlines or other means of transportation. Phone cancellations are not accepted. Send written cancellation or substitution to: meetinginfo@snmmi.org
